Please fill out questionnaire below and then email to me. Once I receive your email, I will let you know your place in my queue and the approximate time that I can get started on your blog. Once I'm ready to begin designing, I will send you an invoice via Paypal, upon prepayment, I will begin your design.

- The name and tagline of your blog.
- Up to 2 or 3 colors you would like to see on your blog.
- An image or picture that represents your blog that you might want to be incorporated into your design.
- Your login and password.
- Your email address
- The package name or number that you would like to purchase.
